Even after reasonable precautions are taken, accidents and illnesses may occur to the participants of the Woolwich Swim Team, for which the Woolwich Swim Team and its volunteers, officers, directors, coaches and instructors cannot assume responsibility. By checking this box, I hereby release and forever discharge the Woolwich Swim Team, its volunteers, officers, directors, coaches, instructors and their heirs from any and all claims arising as a result of my child’s or children’s participation in the Woolwich Swim Team programme, including but not limited to, those for medical, hospital or dental services, save to the extent that such accidents or illnesses may be covered by any insurance which the Woolwich Swim Team has in place. I also hereby consent to the use of my child's name(s) and photographs in newsletters, news reports and websites which promote the Woolwich Swim Team.

In consideration of the opportunity to participate in any Woolwich Swim Team practices, I acknowledge and agree to the following terms:

1. My swimmer(s) will comply with all rules and regulations of Woolwich Swim Team, Government of Canada, Province of Ontario and Public Health concerning matters of health and safety and specifically as they relate to COVID-19 or such other communicable disease or contagion while at Woolwich Swim Team practices.

2. My swimmers(s) may be removed from or prevented from participating in Woolwich Swim Team practices if they fail to comply with all rules and regulations of Woolwich Swim Team, Government of Canada, Province of Ontario and Public Health concerning matters of health and safety and specifically as they relate to COVID-19 or such other communicable disease or contagion, in the sole determination of the Woolwich Swim Team management or coaches.

3. My swimmer(s) will NOT attend a Woolwich Swim Team practice if they did NOT pass the Ontario Government COVID-19 School and Child Care Screening the day of swim team practice.

4. My swimmer(s) will maintain physical distancing measures while at Woolwich Swim Team practices.

5. My swimmer(s) will wear a face covering unless entering and exiting the pool, as instructed by Woolwich Swim Team management and coaches.

6. If my swimmer(s) experiences any symptoms of COVID-19, have tested positive without a subsequent negative text, or have been on contact with anybody who has tested positive within the last 14 days of attending a Woolwich Swim Team practice, I will notify the Woolwich Swim Team management immediately to ensure proper contact tracking and isolation.
